Current high school students and any college transfer interested in pursuing an opportunity as a student-athlete at the University of Memphis must complete and return the following forms.

• Complete the U of M Admissions Application online.
  Please indicate in the occupation section that you are a student-athlete.
  
  
• Complete the Immunization Form.
  Each student must show proof of two doses of the MMR vaccine and Varicella vaccine or proof of immunity.
  
  
• Complete the Hepatitis/Meningitis Form.
  
  

Forms must be on file with the University before a student can register for classes.

Mail/Fax Hepatitis form to your Head Coach.

In addition to the above, International Students must also complete the following:

• World Education Services International Academic Credential Evaluation
  In order to be admitted, the Office of Admissions requires an evaluation completed through World Education Services. Utilize the above link to learn more about this evaluation process.
  
  
• TOEFL
  In order to be admitted, the Office of Admissions requires international students from non-speaking English countries to take the TOEFL exam. Official scores must be sent to the University of Memphis by using code 1459. The minimum required scores are 500 (paper), 173 (computer) and 61 (IBT).
  
  
• Scholarship Tax
  International students may be responsible to pay a tax on their scholarship each semester. This tax cannot be paid by athletic scholarship; the student-athlete would be responsible for this cost.
