APPLICATION PROCESS
Doctor of Philosophy (Ph.D.)
Please familiarize yourself with all items on this page before submitting your application.
Items 1 below provides instructions for completing the University of Memphis Graduate
or International application form.
Items 2-5 describe the required documents necessary for admission to our School and
where they should be mailed.
Item 6 provides you with contact information for a member of our faculty who will
serve to answer questions that you may have regarding the specific graduate degree
program for which you are applying.
1. Application Form
Please submit your application on-line.
Select ‘Graduate Application’ ( U.S. residents only) or 'International Application'
(non U.S. residents only). When asked, create a login ID and password; then simply
follow the directions to complete your application. The application fee is $35.00
for domestic students (U.S. citizens) and $60.00 for international students.
2. Graduate Record Examination (General Test)
We require that you take the Graduate Record Examination (GRE). Please have the Educational Testing Service (ETS) send your scores to The University of Memphis (ETS Code 1459).
3. Official Transcripts
Please have all undergraduate and graduate transcripts mailed directly to: Office of Graduate Admissions, The University of Memphis, Wilder Tower 200, Memphis, Tennessee 38152-3370.

4. Letters of Recommendation
Three letters of recommendation from individuals familiar with your academic background
and aptitude for Ph.D. work within Communication Sciences and Disorders. We do not
have a specific template/form for letters of recommendation.
5. Resumé/Vitae
The resumé/vitae should include a description of all educational and work experience
and a letter describing your research interests and professional goals.
6. Contact Information
Please provide your current contact information (preferred e-mail address and telephone
number) in order to enhance communication in the application process.
