Be sure to include information denoted with the *. We do not issue permits for ASTL, NURS, TEAE, TELC, UNIV, or any course with section 410, M50 or U50. Please go to the registrar's website for the contacts of those permits. ( ♦ Permit requests for COMM 7110 and PRST courses must use the form accessible at this link.)
