Viewing and Printing Instructions

To view the PDF version of our forms, you must have Adobe Acrobat Reader on your computer. If you do not already have this, click here to download the free software.

Each form will have a "Print This Form" icon. Please use this icon instead of the printer icon for your Internet browser. If any of these forms do not print on one or two pages (when applicable), go to the Page Setup box under the File menu of your browser (i.e., Netscape, Internet Explorer, Mozilla Firefox or Safari) and change your page margins to the following specifications: 0.30 for all margins (L,R,T,B); Font Size: 11. You can also increase or decrease the text size of your browser so all the text fields will fit within the margin specifications.

REMINDER: Most of the Graduate School Forms will ask for a University Banner ID number (U-Number). When you log into SSB Banner on the Spectrum Portal "Student Tab", the U-number is in the top right-hand corner on most of the pages you will navigate.
