REQUIRED GRADUATION FORMS (See table below for submission deadlines).
1. Intent to Graduate Card
You will be asked to login with your official University of Memphis UUID
username and password (NOTE: this is your e-mail username and password, not your Student U-Number). Once you have
completed the form, click on the "Preview" button at the bottom of the page. If your
information is correct, click the “Submit” button. You will receive a confirmation
page.
If you are completing requirements for more than one degree program and/or
graduate certificate, you must also submit Intent to Graduate Forms for each program.
After completing and submitting the first form, go back to the form but do NOT login again. Look for the section at the bottom of the web site labeled "For Graduate and Law Students Only". Click on the link in the first bullet item. That should take you to another form
for you to complete and submit.
NOTE: FCBE Master's Students -- Proceed to the following web site to complete and submit the Master's Degree Candidacy Form After submitting the Intent
to Graduate Form.
2. Master's Degree Candidacy Form -- Click "here" for instructions. 3. Doctoral Degree Candidacy Form -- Click "here" for instructions. 4. Graduate Certificate Program Candidacy Form 5. Course Substitution Form (Attach to candidacy form, if needed.) 6. Cap & Gown Order Form (Complete only if you are participating in the graduation ceremony.)
Before graduate students can be officially admitted to degree candidacy, they must
satisfy the following requirements:
- Students completing masters, doctoral, or one of our 21 certificate programs must
submit an Intent to Graduate form to the Graduate School by the deadline listed below in the semester all degree
or all certificate requirements will be completed.
- Master's and doctoral students must submit the appropriate Degree Candidacy form to their respective College Director of Graduate Studies by the deadline listed below. Please read carefully the instructions for completing these forms.
- Students completing a one of our certificate programs must also submit an Intent to Graduate form and a Graduate Certificate Candidacy form to their respective College Director of Graduate Studies by the deadline.
- Students completing an Education Specialist degree may substitute the College of Education, Health and Human Sciences Education Specialist Program of Studies
and Candidacy form for our required form. The Intent to Graduate form will be the only item necessary
to submit for graduation. If you have not completed the EdS program of studies, it should be submitted to the
College Director of Graduate Studies by the deadline.

NOTE: Both the Intent to Graduate Form and candidacy form must be submitted to the appropriate
offices by the specified deadlines. Candidacy forms received by the College Director
of Graduate Studies after the stated deadline will not be approved or forwarded to the Graduate School for processing; therefore, we strongly
recommended that you submit the candidacy form to your department chair at least one
week prior to the college directors' deadline.
Students will be certified for graduation only when all requirements are met, including
the removal of all "I" or "IP" grades from previous semesters and the successful completion
of comprehensive examination requirements.
If a student earns an "I" or "IP" grade in any coursework listed on the candidacy
form, your degree will not be awarded. Students will be notified of their disqualification.
It will be your responsibility to file a new Intent to Graduate Form for the semester
in which all degree requirements will be completed.
Please notify the Graduate School Academic Advisor if you officially change your name or submit an address change with the Registrar's
Office.
