Registration Process:

1. Complete and submit the on-line registration form on the National EYH web site.
   
   
2. There is a $15 registration fee per participant - lunch is included. Parents who register to attend the parents workshops must pay a separate registration fee.  Please use our secure online payment system to pay for each participant. The system only allows you to pay for only one participant at a time. The online payment system asks you to enter "Quantity" on the first screen. Please keep "Quantity" as "1" as the system will not let you proceed with your payment otherwise. Even if you are paying for more than one registration, please enter the payment information for each participant separately.If you need to pay for a large group (5 or more), please contact Shelia Moses <srmoses@memphis.edu> (901) 678-4933 for other payment arrangements.
   
   Payments must be made within 48 hours of online registrations. If no payment is made after 48 hours of registration, participant's registration will be deleted and the reserved spots in chosen workshops will be released.
   
   

3. Please fill out and either email a saved copy of the permission form to Ms. Shelia Moses <srmoses@memphis.edu> or send a printed copy to:

   Important: You need the Adobe Acrobat Reader to view the form.  If you don't have the Reader installed, you can download a free copy from Adobe.
   
   
4. Once forms and fees are received, confirmation will be sent notifying you of your enrollment in the UofM EYH conference. Specific Workshops information will be provided to the participants during their registration the day of the event.

Please do not submit a permission form until you have completed registration through the National EYH web site.  Registrations will not be official until the online registration, permission form, and payment have all been received. To ensure your spot in the selected workshops, all forms and fees must be received by September 11^th. Space is limited. Please register early.

Registration Time: 8:00 A.M.

Closing Time: 3:00 P.M.

Registration and Closing Location: The University Center (UC) located on the Alumni Mall on the University of Memphis main campus. See campus map for location of this building (the red star marks the building).
