Your Name
In order to change your name on your records at the University of Memphis, complete
the Request for Name Change on Official University Record form (pdf). Submit this form and any required supporting documents to the Office
of the Registrar.
You may submit your name change request in one of the following ways:
- Fax to: (901) 678-3249
- Mail to:

- In Person: 003 Wilder Tower (A picture ID is required.)
If you need more information about changing your name, call or e-mail the designated
contact for Student Name Changes.
Your Social Security Number
You may change your social security number (SSN) on your academic records by establishing
your identity and proof of the SSN change. You may do this:
- In Person. Go to Student and Faculty Services, 003 Wilder Tower. Have your SSN card and a photo
ID with you.
- By FAX. Dial (901) 678-3249. FAX a letter of request, a copy of your SSN card, and any other
supporting documentation.
- By Mail. Send a letter of request, a copy of your SSN card, and any other supporting documentation
to:

Your Address Information
You may view all of your addresses in Student Self Service.
You may update all of your addresses, except your Permanent address and your Campus Box, in Self Service.
Note: Every 90 days, the University verifies student addresses with the US Postal Service
(USPS). Your U of M Permanent, Mailing, and/or Local addresses may be updated from
the USPS file. You might notice slight format changes in your addresses based on this
update.

Permanent Address
Initially, your permanent address is entered by University personnel based on your application for admission. It is used as one of the factors in determining your residency classification for
fee purposes. Think of it as your "home" address--where you reside when you are not enrolled at
the University.
Domestic Students
- Your Permanent address is where you reside when not currently enrolled or when classes
are not in session. It must be a physical location with a valid USPS mailing address. For example, if you are from Jackson, TN and attend the U of M, but return to Jackson
for breaks and summer, then your Jackson address is your Permanent address.
International Students
- Your Permanent address must be your out-of-country, permanent, physical home address
and not a post office box.
Note About Residency: Changing your Permanent address will not necessarily change your residency classification
for fee purposes. Your classification as in-state or out-of-state for fee purposes
is made in the Office of Admissions, based on the regulations and guidelines of the
Tennessee Board of Regents. If, for any reason, you have a question about residency
classification, you must request that Admissions review your residency status before
classes begin.
Updating Your Permanent Address
To update your Permanent address, complete and submit a Change Permanent Address form.

Campus Box
This is a campus PO box that you rent through Mail Services. If you have a Campus
Box, this address receives top priority when the University mails you important information.
Updating Your Campus Box
You cannot update your Campus Box. However, when you no longer use or rent the box,
you should contact Mail Services to:
- request that the box be closed.
- leave a forwarding address.

Mailing Address
When the University mails you important information, this address receives high priority--1st
priority if you have no Campus Box address, and 2nd if you do. It does not necessarily
have to be a physical location; it can be a PO Box.
Domestic Students
If you live at home and attend the U of M, your Mailing address can be the same as
your Permanent, but it does not have to be. You can use a PO Box or a temporary local
address (an apartment or house), if you are not living at your Permanent address.
International Students
If you use a PO Box as a Mailing address and do not live in a Residence Hall, you
MUST also supply a Local address (see below).
Student Family Housing
Use your Student Family Housing address as your Mailing address.
Updating Your Mailing Address
Use Self Service to update your Mailing address. Go to your portal Student tab; click
on the "Update Addresses and Phones" link in the Personal Information channel.

Local Address
This is your Memphis area address--where you live while enrolled. It must be a physical
address, not a PO Box.
Domestic Students
You may have a Local address if you do not live at either your Permanent or Mailing
address.
International Students
You must enter and maintain a Local address if your Local residence does not match your Mailing
address.
Updating Your Local Address
Use Self Service to update your Local address. Go to your portal Student tab; click on the "Update Addresses and Phones" link in
the Personal Information channel.

Next-of-Kin Address
You can maintain two Next-of-Kin addresses for relatives the University can contact.

the Personal Information channel.
Note About Updating: Self Service allows you to update the addresses only. If you need to update the person and/or relationship listed, you will have to contact
Student and Faculty Services at (901) 678-2810, 003 Wilder Tower.

Emergency Contact Address
The address of someone the University can contact in an emergency.

Use Self Service to update your Emergency Contact person/address. Go to your portal Student tab; click on the "Update Emergency Contacts" link in the
Personal Information channel.
