How do you apply?

Application must be made to the Master of Public Health Program and the Graduate School.

Materials to be sent to the MPH Program:

MPH Application (apply to either the MPH Campus Program OR the MPH Online Program)

Statement of Purpose (400-500 words)

Letters of Recommendation from three references (at least one from former or current faculty member). Please attach the Reference Letter Form

Curriculum Vitae

Copy of GRE Scores (Official copy should be submitted to the University of Memphis Graduate School)
