This is your guide for applying for graduation. Please follow directions carefully. Complete and accurate information is essential if you want to graduate on time and have a diploma with no errors. The Degree Application Form must be filled out completely. The name you use on the Degree Application Form will also appear on your diploma. This name must be the same name used by the Records Office. If you have changed your name go to Room 003 of Wilder Tower to have it corrected in their database.

Firm Deadlines For Applying for Graduation

• July 22, 2009 - for December 2009 graduation
• November 11, 2009 - for May 2010 graduation

Procedures for Applying for Graduation

• Submit the online intent to graduate form
  
  
• Students majoring in:
  
  
  • ITGS: Society Services (Non-Licensure)
  • Sports and Leisure Studies
  • Health and Human Performance with a concentration in Dietetics and Exercise Sports Sciences
  
  must submit a copy of their degree sheet from their advisor.
  
  
• In two weeks of submitting your application, an email which contains important documents requiring your signature will be sent to your University of Memphis email address. Not responding to this email in a timely fashion can result in you not graduating.
