DocuSign / Dynamic Forms
DocuSign / Dynamic Forms provides a simple way for its users to take any paper-based form and quickly build a sophisticated interactive electronic form that can be published to the internet. Key features of DocuSign / Dynamic Forms:
• Electronic Signatures - Allow a single user or multiple users to sign a form electronically
• Single Sign On - Users who have access to the myMemphis portal should be able to access DocuSign / Dynamic Forms using Single Sign On; other users will be sent to the DocuSign / Dynamic Forms website directly.
• Data/Image Exchange - Downloading form images from DocuSign / Dynamic Forms to our document retention system is possible if the form is essential for document retention purposes. DocuSign / Dynamic Forms keeps documents seven years before purging.
• User Portal - Allows users to visit and view pending forms or see a history of previously signed forms.
Who Can Request?
Faculty and Staff can request access to develop forms for departmental webpages using DocuSign / Dynamic Forms. IT System Access and Security will enable user accounts upon request for access via the Helpdesk.

What Information Do I Need to Fulfill Service Request?
Select "DocuSign / Dynamic Forms" from the list of available Software, select your Affiliation (Faculty or Staff), enter your issue in the Information field.
