Masters Forms

Most forms required for graduation will be completed during IDT 7095, the capstone course for both masters program tracks. However, a list of required forms is provided for your convenience. 

Add the Certificate Program

Complete the Academic Program Update Form > to add the EdTech or Design and Development graduate certificate program to your master’s degree program (all masters students earn one of these certificates with their degree program).

If you needed to substitute one course in your program with another, complete the Course Substitution Form >

Graduate School Candidacy Form

In IDT 7095 you will Complete the Graduate School Candidacy Form for Masters Degree students > (your instructor for IDT 7095 is your major professor, your graduate studies coordinator is the College of Education: MS Instruction and Curriculum Leadership, and your College Director of Graduate Studies is through the College of Education). 

Also complete the Graduate School Candidacy Form for Certificate students > (your certificate coordinator is the College of Education: College of Education: Certificate in Instructional Design and Technology-Design and Development or -K-12 Educational Tech, and your College Director of Graduate Studies is through the College of Education).

Comprehensive Exam Results Form

During IDT 70795, students work with community clients to develop instructional products that meet organizational needs. The collective assignments become the written portion of the comprehensive exam. Students also complete a 30-minute, recorded oral defense during the course. Both the written and oral portions of this exam constitute the Capstone project and defense. Following these defenses, your instructor will complete and submit the Comprehensive Exam Results Form >.
