Required Nursing Data Documents

Nursing students are required throughout the RN to BSN program to be current in essential vaccinations, and to maintain certifications and liability insurance to protect themselves and clients in their care, and to meet clinical site requirements. After admission, students will receive directions via email to purchase an initial bundle package through Complio, the University of Memphis health data management partner site. Documents must be maintained and updated as needed through Complio.

Immunizations and Required Documents

  1. Authorization for Release of Student Information (form located on Complio)
  2. Student Signature form (form located on Complio)
  3. Health Examination Form completed by a licensed health care provider. (Physical form located on Complio)
  4. Criminal Background Check: completed prior to beginning program and annually thereafter.
  5. Urine Drug Screen: All students are required to submit results of a 10-panel drug screen prior to beginning of the program and annually thereafter.
  6. CPR certification (ONLY AHA BLS Provider is accepted)
  7. Immunizations

    1. Proof of COVID Vaccine: Students are required to be fully vaccinated against the COVID-19 virus and must submit copies of their COVID vaccine card prior to the start of their first term. Evidence of 2 doses of the Pfizer, Moderna or Novavax vaccine, or 1 dose of the Janssen must be shown on the COVID vaccine card.
    2. Hepatitis B: A) 3 doses of the Hepatitis B Vaccine or B) Positive Hepatitis B Titer.  If your titer is not immune, then you must submit three repeat doses dated after the titer or C) Hepatitis B Declination.
    3. Influenza vaccine: an annual flu shot is required between October and December unless you have an allergy to eggs, in which case you will need to upload a waiver stating such signed by your health provider.
    4. MMR: A) 2 doses of the MMR Vaccine or B) Positive titers for Measles, Mumps, and Rubella
    5. Polio vaccine PPD (TB skin test) An annual negative
    6. PPD test for tuberculosis, statement from a health care provider attesting to a negative tuberculosis status or a negative chest x-ray every two years.
    7. Tetanus, Diphtheria, Pertussis (Tdap): must submit proof of your Tdap within the last 10 years.
    8. Varicella: A) Evidence of a positive varicella titer (date and result) or B) verification of vaccine series (two). If titer is negative or equivalent, documentation of two varicella vaccines given no less than 4-8 eight weeks apart is required.

  8. Malpractice Insurance: All nursing students are required to have current malpractice insurance throughout the program.  BSN students are charged for insurance upon admission and every spring.  RN-BSN students who have personal professional malpractice insurance may submit a current insurance certificate as proof of coverage (professional liability limits at $1MM/$5MM). Otherwise, the LCON will charge RN students for professional student malpractice insurance annually through the UofM Bursar's Office; please note that this insurance only covers practice in the academic clinical setting.
