Giving Forms
Review the Giving Brochure and see all the possibilities you have to make a difference. You can select up to 3 different organizations to make a donation.
Use the online giving form to select the organization(s) you would like to make donations. The form will populate the selection box with the charitable organizations 6-digit code, calculate your total donations, has a print option and will send an e-mail confirmation notice to your university e-mail for tax purposes.
If you prefer to use the PDF version of the pledge form, look up your charitable organization's 6-digit code in the Giving Brochure. Click on the link below to print the forms. Completed paper forms should be returned to Paige Richmond, Human Resources, 171 Administration Building no later than November 15, 2018.
