Please note that this form only requests permits for classes and does not register you for the course.  The College of Professional & Liberal Studies does not offer registration for graduate courses after the first week of classes.

Please complete all parts of our permit request form. You will receive a response when your permit is ready.  Requests sent without CRNs WILL NOT be processed by our office.
