Service Description: 

Tennessee requires state public institutions of higher education to verify that students have received mandated immunizations and health requirements. You may upload immunization documentation at https://umwa.memphis.edu/immunizations.   

Who may request (or are eligible) for this service? 

Requests can be made by Students admitted to the University.
