Campus Card
- The University of Memphis issues each student, faculty and staff member an identification
card that bears their name and image, referred to as your Campus Card. This permanent
identification card is used as the primary campus-wide method of determining privileges
and accesses.

If you are planning an event for the School of Public Health community (students,
faculty/staff, alumni, etc.), you must complete the Event Request form at least 2
weeks prior to the event date.
If you need food ordered for an event on your behalf, you must complete the Food Request
Form at least 2 weeks prior to the event date.

UofM student, faculty, and staff public printers are provided in the
Learning Commons computer areas on all four floors of McWherter Library, as well as the
sandbox.
On-campus printing may be processed through desktops, laptops, and other devices.

Filling out Sponsor forms frequently requires information about the University's authorized
officials, registration numbers and codes, Congressional Districts, and Tax ID number.
The University of Memphis manages human subject research protocol review through
Cayuse IRB.
Training in the ethical conduct of research is required for all SPH students and faculty
participating in human subjects research. Training courses are available through
CITI.
